TOPAL Liposuction and Smart Lipo - A Fully Awake Procedure

The tumescent in office power assisted laser lipolysis is a new technique that Dr. Ayman El-Attar, founder of Derma Laser Centers has introduced to the medical community in 2008. Dr. El-Attar is lecturing about this procedure throughout the world.
This new technique combines the best of use of safe in-office tumescent technique, the PAL system by MicroAire, and using laser lipolysis device at the end of the procedure. Laser lipolysis was first approved in the U.S. in 2006, but had their inherent limitation on the amount of fat that can be removed in one session. This made more physicians skeptical about their efficacy as a stand alone tool for patient seeking liposuction.
Dr. El-Attar explains that most patients seeking this new technology might not fit the perfect selection criteria due to their body mass index. TOPAL™ offers a new horizon of utilizing laser lipolysis in conjunction with power assisted tools.
After performing over three hundred procedures utilizing TOPAL™ our patient satisfaction rate is 98% and our touch up rate is 3%. These numbers are both much better figures than those for traditional liposuction or laser assisted lipolysis alone.
The new technique achieves dramatic results by debulking the fat using the PAL system, then delivering Nd:YAG laser energy through the small skin entry to heat the fatty deposits and the skin up to 42° C. The laser while destroying more fat cells for even better results will simultaneously induce collagen contraction and more reticulin formation leading to progressive skin tightening over the upcoming twelve months.
The TOPAL™ technique offers a faster, safer and less invasive alternative to other liposuction methods, with fewer side effects and a shorter recovery time. The procedure is virtually painless and requires no sedation at all. Patients can have their abdomen and back done in a four hour session. The patient is able to return to work within 48 hours with minimal post operative pain. The skin continues to tighten over 12 months after the procedure. Unlike traditional liposuction, the TOPAL™ procedure makes it possible to quickly lose a few sizes, have no loose skin after the procedure and feel well enough to go back to work in just a few days. “This procedure will largely replace traditional liposuction,” Dr. El-Attar explained. “The old way was a more aggressive procedure performed under general anesthesia. TOPAL™ is done in the office under local or (tumescent) anesthesia, uses a PAL (power assisted device) for comfortable and significant fat removal, creating less pain and resulting in less downtime and faster recovery.” Jardin Medical Spa applauds those who exercise and eat a balanced diet. How does the TOPAL ™ procedure work?
After one hour of applying the tumescent anesthesia, a very small cannula (tube) approximately 2.0 mm in diameter is inserted into the skin. The cannula vibrates at a high rate back and forth breaking all the fibrous septae and suctioning fat cells. After wards using a 600 micron fiber the laser’s energy directly to the fat cells, causing them to rupture and easily drain away. The laser energy also interacts with the dermis, resulting in collagen remodeling.
Due to the cannula’s small size, TOPAL™ is a minimally invasive procedure and can be performed under local anesthesia. The laser causes small blood vessels to coagulate immediately on contact, resulting in less bleeding, swelling and bruising – and a quicker recovery time than conventional liposuction. Because general anesthesia is unnecessary, the procedure is very safe, and can be performed safely in an office setting without the need for additional costs of general anesthesia, and a hospital operating room.
Who is a good candidate for the TOPAL ™ procedure procedure?
If you have areas of fat that will not respond to diet and exercise, then you’re a candidate for Laser lipolysis. Ideal candidates are in good health, of normal body weight and have realistic expectations for the results. Also patients who are not satisfied with their previous results from traditional liposuction will benefit from laser lipolysis, to adjust their irregularities.
What body regions can be treated with TOPAL ™ procedure?
Smartlipo can be a great adjunct to tumescent liposuction procedures, although it does not replace conventional liposuction. It is ideal for treating small areas of the face, neck, arms, breasts (gynecomastia), and abdominal areas and areas of loose and flabby skin.
How many treatments will I need?
For most patients, typically only one treatment is required. The session may last from 45 minutes to one hour for each area treated. Any touch ups that maybe required in about ten percent of the patients, are included in the initial cost of the procedure.
What do I have to do after the treatment?
After the treatment you will need to wear a compression garment for several days to several weeks, depending on the body area being treated. Rest is recommended after the procedure for one day and activity may be resumed gradually. Refrain from hot tubs or strenuous exercise for 2 weeks. You will be advised by Dr. El-Attar to start your lymphatic drainage which helps your recovery with best results obtained.
When can I expect to see results?
Results can be seen as soon as a week with continued improvement over several months.
Does laser lipolysis provide lasting results?
Yes! Adults do not create new fat cells. When a person gains weight, the fat cells expand. The procedure permanently removes the fat cells. Should you gain weight, most fat will be deposited in areas where the procedure was not performed.
Dr. El-Attar is a board certified physician and surgeon who is dedicated to the art and science of cosmetic and laser Surgery. He teaches and lectures about the most recent advances in cosmetic laser surgery. He graduated from Alexandria Medical School with honors in 1987. He completed residencies in both General Surgery and Family Medicine, and obtained a Master’s Degree of Surgery in 1992. He was an assistant lecturer of surgery at Alexandria University, and a visiting instructor of surgery at the Medical College of Ohio. He then finalized his Ph.D. thesis in 1998. Dr. El-Attar is an active member of the following professional organizations: Fellow of the American Society of Laser Medicine and Surgery, and the American Academy of Family Medicine; Associate of The American Academy of Cosmetic Surgery; and Member of the American Society of Hair Restoration Surgery, the American Academy of Aesthetic Medicine, the American Society of Aesthetic Mesotherapy, the American Academy of Phlebology, the Canadian Academy of Aesthetic Medicine and the American Medical Association.
